Week 5 (Jason Guss)

This week consisted of a varied schedule of shadowing opportunities.  On Monday I shadowed Dr. Florence Yu who is pre/peri op doctor that works on 72nd street. This was a new experience because these doctors chief job is to clear patients for surgery at HSS and so there check up of the patient is a bit different. They want to confirm that the patient is not at a considerable risk for blood clot, bleeding out, or infection during/after surgery.  Their management of the patient is to minimize and prevent any chance of these occurring. HSS historically has great patient outcomes and low infection rates and this is a result of good pre/peri op management.

I also rounded in the neonatal ICU this week. This was surprisingly an uplifting experience because the majority of these babies are on their way to recovery. There was only one case that was very sad that I will not go into detail about.  The majority of problems with these children involved their respiratory or circulatory systems.

I also did the ER rotation this week and to no surprise found that it was no where near as crazy as they make it seem on the TV shows. A lot of the cases are not that serious and when this is the case the patients are cleared to follow or triaged for longer. The most exciting/disturbing case that I witnessed was a woman who was at memorial sloan kettering and then started having a stroke while being transported. She was brought right to the ER and she began to have a seizure in the ER. It was crazy to witness this happen and to watch the large number of doctors, nurses, and EMS that all had to communicate quickly and effectively to help this patient.  I was really impressed with how fast the doctors have to act with such limited information available.  The woman could not talk at all and barely responded to human contact.
